What to expect

No one can promise a trial will help you personally — that is what the research is trying to find out.

What participation involves

Not stated in the study description — ask the study team about visits and procedures.

Compensation
Not stated
  • Study-related exams and procedures are typically provided at no cost. This is how many trials work in general — confirm the details for this study with the study team.
  • Participants are monitored closely by the research team throughout the study.
  • Some participants may receive access to an investigational treatment, which is still being studied.
  • Your participation helps researchers learn whether this approach could help future patients.

About this study

In plain language

This phase 2 study tests mFOLFOX chemotherapy and a short course of radiation before surgery for rectal cancer. It is for adults whose cancer has not spread to distant parts of the body and can be removed with surgery.

This is phase 2 trial of neoadjuvant therapy and short-course radiotherapy in resectable rectal cancer.

Compensation is read from the study's own description and may be missing — always confirm with the study team.

Eligibility

Who can join

  • Be at least 18 years old and able to care for yourself for most of the day.
  • Have rectal adenocarcinoma confirmed by a tissue sample within 90 days before signing up. The tumor must meet the study’s location rules within the rectum.
  • Have cancer that has grown into or through the rectum’s outer layers, or spread to nearby lymph nodes, but not to distant body parts.
  • Have the required exams and scans to check the cancer’s stage, including a colonoscopy unless the tumor blocks the bowel. Required exams and scans must be done within the study’s time limits.
  • Have blood counts, liver function, and kidney function that meet the study’s limits.
  • Have a surgeon agree that surgery could remove the cancer with the goal of curing it.
  • If able to become pregnant, have a negative blood pregnancy test within 7 days before chemotherapy. These patients and men must use accepted birth control during treatment and for 3 months after chemotherapy.
  • Be able to understand the study and willing to sign a consent form.

Who can't join

  • Have had radiation that would overlap unsafely with the study’s radiation.
  • Have serious heart disease or a lung or breathing condition that makes the study chemotherapy unsafe.
  • Have a severe infection that is not under control.
  • Have had major surgery within 28 days before joining, except surgery to make an opening that diverts stool into a bag.
  • Have inflammatory bowel disease, such as Crohn’s disease or ulcerative colitis, that has needed major treatment and makes study treatment unsafe.
  • Have a known allergy to fluorouracil, leucovorin, or oxaliplatin, or a known lack of DPD, a substance the body uses to break down fluorouracil.
  • Have cancer that has spread to distant body parts, or be pregnant or breastfeeding.
  • Have a medical, mental health, or social condition that could increase risk or make it hard to follow study requirements.

Simplified for readability. The official criteria below are what the study team uses.

Official eligibility criteria

Inclusion Criteria: * Pathologic diagnosis of adenocarcinoma of the rectum (diagnosis by tissue biopsy) within 90 days prior to registration. At least a portion of the tumor must be located below the peritoneal reflection or begin within 12 cm of the anal verge on flexible endoscopy * Clinically staged (AJCC 8th ed.) T3-4 N0 M0 or T any N1-2 M0 based upon the following minimum diagnostic workup: * Colonoscopy, unless patient presents with an obstructing lesion * Within 30 days before initiating MFOLFOX6 treatment: * History/physical examination * Imaging to exclude distant metastases: either contrast-enhanced CT of the chest, abdomen, and pelvis; or whole-body PET-CT; or MRI * Pelvic MRI (preferred) or transrectal ultrasound (TRUS) for T staging Note: Patients may have initiated standard mFOLFOX6 treatment before study registration provided that they met the above criteria before initiating treatment and can feasibly continue to CRT according to the timeline described in Section * ECOG Performance Status ≤2 * Age ≥ 18 years * Adequate bone marrow function defined as follows: * Absolute neutrophil count (ANC) ≥ 1,200 cells/mm3 * Platelets ≥ 100,000 cells/mm3 * Hemoglobin ≥ 8.0 g/dL (Note: The use of transfusion or other intervention to achieve Hgb ≥8.0 g/dL is acceptable.) * Adequate liver and renal function defined as follows: * AST and alkaline phosphatase \< 2.5 x upper limit of normal (ULN) * Bilirubin ≤ 2.5 ULN * Calculated creatinine clearance (CrCl) \> 30 mL/min using Cockcroft-Gault formula as calculated by the standard Cockcroft-Gault equation using age, actual weight, creatinine, and gender * Must be deemed a candidate for curative resection by the surgical oncologist who will be performing the operation * Women of childbearing potential (WCBP) must have a negative serum pregnancy test performed within 7 days prior to the start of chemotherapy. * WCBP and men must agree to use a medically accepted form of birth control during the treatment and for 3 months following completion of chemotherapy. * Ability to understand and the willingness to sign a written informed consent document. Exclusion Criteria: * Prior RT that would result in unsafe overlap of RT fields with the planned study treatment, per the treating radiation oncologist * Clinically significant cardiac disease, including major cardiac dysfunction, that in the opinion of the treating medical oncologist would preclude them from receiving systemic therapy with 5-fluorouracil, leucovorin or oxaliplatin. * Serious (ie, ≥ grade 3) uncontrolled infection * Pulmonary or respiratory condition that, in the opinion of the treating medical oncologist would preclude them from receiving systemic therapy with 5-fluorouracil, leucovorin or oxaliplatin. * Major surgery within 28 days of study enrollment (other than diverting colostomy) * History of inflammatory bowel disease (eg, Crohn's disease, ulcerative colitis) requiring significant intervention (eg, hospitalization, surgery, immunosuppressive medications) that would, in the opinion of the investigator, preclude study therapy * Prior known allergic reaction to 5-fluorouracil, leucovorin, or oxaliplatin * Known dipyrimidine dehydrogenase deficiency (DPD) * Any evidence of distant metastases (M1) * Pregnant or breast feeding * Medical, psychological, or social condition that, in the opinion of the investigator, may increase the patient's risk or limit the patient's adherence with study requirements
