What to expect

No one can promise a trial will help you personally — that is what the research is trying to find out.

What participation involves

Not stated in the study description — ask the study team about visits and procedures.

Compensation
Not stated
  • Study-related exams and procedures are typically provided at no cost. This is how many trials work in general — confirm the details for this study with the study team.
  • Participants are monitored closely by the research team throughout the study.
  • Some participants may receive access to an investigational treatment, which is still being studied.
  • Your participation helps researchers learn whether this approach could help future patients.

About this study

In plain language

This study tests an injection called 177Lu-rhPSMA-10.1 for prostate cancer that has spread and keeps growing despite very low testosterone levels. The cancer must show a protein called PSMA on a scan. The study checks the dose, safety, where the radiation goes in the body, and how well the treatment works.

To determine the dose, safety, radiation dosimetry and efficacy of 177Lu-rhPSMA-10.1 in participants with PSMA-expressing metastatic castrate resistant prostate cancer.

Compensation is read from the study's own description and may be missing — always confirm with the study team.

Eligibility

Who can join

  • Men age 18 or older with prostate adenocarcinoma, a type of cancer confirmed by a tissue test.
  • Testosterone below 50 ng/dL after surgery or ongoing hormone treatment. Scans must show cancer, and a PSMA PET/CT scan must show PSMA.
  • Wait at least 4 weeks after the last cancer treatment, or longer if the drug stays in the body longer. Certain ongoing hormone treatments are allowed.
  • Most side effects from past treatment must be mild or gone. Hair loss, moderate nerve damage, or moderately frequent urination are allowed. Major surgery must have been at least 12 weeks earlier.
  • Able to care for yourself and be out of bed for most of the day, with an expected life span of at least 6 months.
  • Blood tests must show enough healthy blood cells and good organ function. Patients and their partners must use adequate birth control.
  • Phase 1: Cancer worsened during or after at least one newer hormone-blocking drug, such as abiraterone or enzalutamide, and 1 or 2 courses of taxane chemotherapy.
  • Phase 2: Cancer worsened during or after at least one newer hormone-blocking drug. No past taxane chemotherapy for prostate cancer that resists treatment to lower testosterone.

Who can't join

  • Allergy to the study treatment or scan drug; a large amount of cancer without PSMA; widespread cancer in bone marrow; or pressure, or likely pressure, on the spinal cord.
  • Past blood cancer or cancer spread to the brain or spinal cord; a nerve-and-hormone-cell type of prostate cancer; or another solid cancer that may shorten life or affect study results.
  • Unresolved urine blockage causing kidney swelling; severe urine leakage that prevents safe disposal of radioactive urine; one kidney; a kidney transplant; or kidney-damaging treatment that poses a high risk.
  • An uncontrolled health or mental health problem, unsafe lab results, important heart tracing changes, or a history of serious disease in the lung tissue.
  • Ongoing bisphosphonate drugs for bone treatment; past PSMA-targeted radioactive treatment; or past treatment with strontium-89, samarium-153, rhenium-186, rhenium-188, or radium-223.
  • Past radiation to half the body, to the chest, or to an area covering more than 30% of the bone marrow or kidneys.
  • Both hips replaced or metal that may affect scans or radiation measurements; or unusually high PSMA scan activity in lung tissue.
  • Blood transfusions only to meet study entry rules; or another study using an investigational drug during this study or too recently before entry—within 28 days, or longer for drugs that stay in the body longer.

Simplified for readability. The official criteria below are what the study team uses.

Official eligibility criteria

Inclusion Criteria: 1. Male subjects, 18 years of age or older with histologically confirmed adenocarcinoma of the prostate. 2. Serum testosterone levels \<50 ng/dL (1.73 nmol/L) after surgical or continued chemical castration. 3. Presence of disease target or non target lesions (per RECIST v1.1) on CT/MRI and/or presence of disease on full body 99mTc bone scan performed within 28 days of screening. 4. Positive disease expression of PSMA as confirmed on PSMA PET/CT scan. 5. At least 4 weeks or 5 half-lives (whichever is longer) elapsed between last anti-cancer treatment administration and the initiation of study treatment (except for Luteinising Hormone-releasing Hormone or GnRH). 6. Resolution of all previous treatment related toxicities to CTCAE version 5.0 grade of ≤1 (except for chemotherapy induced alopecia and grade 2 peripheral neuropathy or grade 2 urinary frequency which are allowed). 7. Prior major surgery must be at least 12 weeks prior to study entry. 8. Eastern Cooperative Oncology Group (ECOG) performance status of 0-2 with a life expectancy ≥6 months. 9. Adequate bone marrow reserve and organ function as demonstrated by blood count, and serum biochemistry at baseline. 10. Adequate contraception for patients and their partners. 11. For Phase 1 mCRPC only: Subjects who have experienced disease progression on or after at least 1 NAAD (e.g. abiraterone, enzalutamide) and at least 1 course (but no more than 2 courses) of taxane-based chemotherapy. For Phase 2 mCRPC only: Subjects who have experienced disease progression on or after at least 1 NAAD (e.g. abiraterone, enzalutamide, apalutamide, darolutamide), but have not received previous taxane-based chemotherapy for the treatment of mCRPC. Exclusion Criteria: 1. Known hypersensitivity to the therapeutic or diagnostic IMP or any of its constituents. 2. Presence of significant PSMA-negative disease on ceCT/MRI scan 3. Diffuse marrow infiltration of disease ('superscan' appearance on full body 99mTc bone scan). 4. Symptomatic spinal cord compression, or clinical or radiological findings that are indicative of impending spinal cord compression. 5. Known history of haematological malignancy. 6. Known history of central nervous system (CNS) metastases. 7. Histological findings consistent with neuroendocrine phenotype of prostate cancer. 8. Known history of other solid malignancy that may reduce life expectancy and/or may interfere with disease assessment. 9. Unresolved urinary tract obstruction defined as radiographic evidence of hydronephrosis with or without ureteric stent/nephrostomy. 10. Any uncontrolled significant medical, psychiatric, or surgical condition or laboratory finding that would pose a risk to subject safety or interfere with study participation or interpretation of individual subject results. 11. Ongoing treatment with bisphosphonates for bone-targeted therapy. 12. Severe urinary incontinence that would preclude safe disposal of radioactive urine. 13. Single kidney or renal transplant or any concomitant nephrotoxic therapy that might put the subject at high risk of renal toxicity during the study in the judgement of the investigator. 14.
