- "CAR19PK is a Phase II study evaluating lymphodepleting chemotherapy (age-based fludarabine dosing and cyclophosphamide), followed by infusion of CD19-CAR T cells, in pediatric and young adult patients ≤ 21 years old with relapsed/refractory CD19-positive leukemia."
- "Treatment will include a single course of lymphodepleting chemotherapy followed by CAR T cell infusion."
- "The CAR T cell infusion will include a single infusion of 3x10\^6 CD19-CAR T cells/kg patient weight."
- "This protocol contains a two-part consent process: 1) to proceed with autologous apheresis and 2) to proceed with treatment with lymphodepleting chemotherapy and infusion of the CD19-CAR T cell product."
- Compensation

- Study-related exams and procedures are typically provided at no cost. This is how many trials work in general — confirm the details for this study with the study team.
- Participants are monitored closely by the research team throughout the study.
- Some participants may receive access to an investigational treatment, which is still being studied.
- Your participation helps researchers learn whether this approach could help future patients.
About this study
In plain language
This study tests chemotherapy followed by CAR T cell therapy for people age 21 or younger with CD19-positive leukemia that has come back or has not responded to treatment. CAR T cells are immune cells changed to find cancer cells with a marker called CD19. Researchers will study chemotherapy doses, how the body handles the drugs, how long CAR T cells last, how well treatment works, and side effects.
CAR19PK is a research study evaluating the use of lymphodepleting chemotherapy and chimeric antigen receptor (CAR) T cell therapy, a type of cellular therapy, for the treatment of refractory and/or relapsed leukemia. For this type of therapy, peripheral (circulating) immune cells are collected and then modified so that they can recognize an antigen, which is a particle present on the surface of a cancer cell. The CD19-CAR T cell product will be manufactured at the St. Jude Children's Research Hospital's Good Manufacturing Practice (GMP) facility. The main purpose of this study is to determine: * Evaluate different doses of fludarabine prior CAR T cell infusion * How your body processes fludarabine and cyclophosphamide, * How long the CAR T cells last in the body, * Whether or not treatment with this therapy is effective in treating people with refractory or relapsed leukemia, and * The side effects of this therapy.
Compensation is read from the study's own description and may be missing — always confirm with the study team.
Eligibility
Who can join
- Be age 21 or younger and have leukemia confirmed to have the CD19 marker within 3 months before joining treatment.
- Leukemia must not respond to treatment, have returned at least twice, or have returned after a donor stem cell transplant. A first return may qualify if a donor transplant is needed but is not suitable.
- Have a daily activity score of at least 50 on the study's scale. Expected survival must be more than 12 weeks before cell collection and more than 8 weeks before treatment.
- Have cancer that can be found by testing before treatment.
- If you had a donor stem cell transplant, you must have recovered before cell collection and be at least 3 months past transplant before treatment. You must have no active graft-versus-host disease, when donor cells attack your body, and no donor immune cell infusion in the 28 days before collection or treatment.
- Before treatment, heart, kidney, lung, and liver tests must meet study limits. You must not have a serious abnormal heartbeat.
- Before treatment, you must have recovered from severe sudden side effects of earlier treatment, except those affecting blood cells.
- If you can become pregnant, have a negative blood pregnancy test within 7 days before joining. You must not be making breast milk with plans to breastfeed. If sexually active, agree to use birth control until 6 months after the T cell infusion.
Who can't join
- An immune system disorder you were born with.
- A history of HIV infection.
- A severe bacterial, viral, or fungal infection before cell collection, or a severe uncontrolled infection before treatment.
- A past allergic reaction to products containing mouse proteins.
- A medical reason you cannot receive the study chemotherapy that lowers immune cell levels.
- Before treatment, active leukemia in the brain or spinal fluid classified as CNS-3.
- Before treatment, an active, uncontrolled disease affecting the brain or nerves.
Simplified for readability. The official criteria below are what the study team uses.
