About this study
In plain language
This study tests T1CARE, a program for young adults with type 1 diabetes that addresses social needs that affect health. People are assigned by chance to get support from a Community Health Navigator at Project Access-New Haven or to join a wait list and get their usual care.
Randomized controlled trial with a wait list control arm. Participants will be randomized 1:1 to T1CARE or wait list usual care arm. Participants will be recruited from the Yale Adult and Pediatric Diabetes Centers. Patients assigned to T1CARE will receive support from a Community Health Navigator at Project Access-New Haven.
Compensation is read from the study's own description and may be missing — always confirm with the study team.
