What to expect
No one can promise a trial will help you personally — that is what the research is trying to find out.
What participation involves
Not stated in the study description — ask the study team about visits and procedures.
- Compensation
- Not stated
- Study-related exams and procedures are typically provided at no cost. This is how many trials work in general — confirm the details for this study with the study team.
- Participants are monitored closely by the research team throughout the study.
- Some participants may receive access to an investigational treatment, which is still being studied.
- Your participation helps researchers learn whether this approach could help future patients.
About this study
In plain language
This study tests whether zanubrutinib, sonrotoclax, and obinutuzumab together can help control CLL/SLL, cancers of white blood cells. It is for adults whose CLL/SLL has not been treated before and now needs treatment.
The goal of this clinical research study is to learn if the combination of zanubrutinib, sonrotoclax, and obinutuzumab can help to control previously untreated CLL/SLL.
Compensation is read from the study's own description and may be missing — always confirm with the study team.
Eligibility
Who can join
- Be age 18 or older and have CLL/SLL that meets the study’s rules for needing treatment.
- Have no past CLL/SLL treatment, except certain steroids or CD20 antibody drugs. Special waiting periods and steroid dose limits apply.
- Be able to care for yourself and be up and about for at least half of your waking hours.
- Have liver, kidney, and blood-clotting test results within the study’s limits.
- Have enough platelets, which help stop bleeding; hemoglobin, which carries oxygen; and neutrophils, which fight infection. These levels must meet study rules without recent transfusions or drugs to boost blood cells.
- Be able to swallow tablets and attend all required treatment visits, blood tests, and clinic visits.
- If you can become pregnant, have a negative pregnancy test within 24 hours before the first dose. Use effective birth control during treatment and for at least 18 months afterward.
- If you are a man with a partner who can become pregnant, use condoms or have had a vasectomy during treatment and for at least 18 months afterward. Do not donate eggs or sperm for at least 18 months after the last dose.
Who can't join
- Major surgery or a live-virus vaccine within 4 weeks before treatment starts.
- An uncontrolled infection, HIV, active hepatitis B or C, or active CMV infection. Some people with past hepatitis B may join if testing and preventive treatment meet study rules.
- Uncontrolled immune problems that destroy red blood cells or platelets and need more than 20 mg of prednisone daily, or an equal dose of another steroid.
- Serious uncontrolled heart disease, certain heart rhythm problems, or uncontrolled high blood pressure. A heart attack, stroke, or bleeding inside the skull within 6 months also rules you out.
- Being pregnant or breastfeeding, or planning to breastfeed during the study or within 6 months after treatment ends.
- Taking warfarin, similar blood thinners, or certain drugs that affect how study drugs are broken down. Eating grapefruit, Seville oranges, starfruit, or their listed products within 3 days before treatment also rules you out.
- CLL/SLL in the brain or spinal cord; prolymphocytic leukemia; or past or suspected Richter transformation, when CLL/SLL becomes a faster-growing cancer. Another active cancer may rule you out, though exceptions apply.
- An allergy to study drug ingredients, trouble absorbing drugs through the gut, a known bleeding disorder, or past bleeding without an injury that needed a transfusion or other medical care.
Simplified for readability. The official criteria below are what the study team uses.
Official eligibility criteria
Inclusion Criteria: 1. Patients with a diagnosis of previously untreated CLL/SLL meeting iwCLL 2018 indication for treatment (Note: patients who receive steroids and/or CD20 mAb for cytoreduction in those patients presenting with significantly elevated WBC count or significant adenopathy/organomegaly and those who previously received steroids/CD20 mAb for immune cytopenias are eligible to enroll; Washout of 3 months applies for CD20 mAb and dose of prednisone (or equivalent) should be less than 20 mg/day by day 1 of study initiation) 2. Age greater than or equal to 18 years of age 3. Eastern Cooperative Oncology Group (ECOG) Performance status of 0-2 4. Adequate hepatic function a. Total bilirubin ≤1.5 x upper limit of normal (ULN) or ≤3 x ULN for patients with Gilbert's disease or documented disease involvement of liver (In pts with elevated total bilirubin due to increased indirect bilirubin, pts with direct bilirubin ≤1.5 x ULN are eligible) b. ALT and AST ≤3.0 x ULN, or ≤5.0 x ULN if documented disease involvement of liver 5. Adequate renal function a. Adequate renal function defined by a value ≥30 mL/min determined via estimated GFR calculated according to the CKD-EPI equation 6. Adequate hematologic function a. Platelet count ≥50 x109 /L and hemoglobin ≥8 g/dL (≥80 g/L). Platelet and hemoglobin requirements are independent of transfusions within 7 days of screening assessment and first dose of study drugs. b. Absolute neutrophil count ≥0.75 x 109 /L. Absolute neutrophil count is independent of growth factor support within 7 days of screening assessment and first dose of study drugs. 7. Adequate coagulation function a. INR ≤1.5 x ULN and aPTT ≤1.5 x ULN 8. Ability to swallow tablets and comply with outpatient treatment, laboratory monitoring, and required clinic visit for the duration of study participation 9. Women of childbearing potential must have a negative serum or urine beta human chorionic gonadotropin (β-hCG) pregnancy test result within 24 hours prior to the first dose of study drugs and must agree to use an effective contraception method (combined hormonal contraception, progestogenonly hormonal contraception associated with inhibition of ovulation, intrauterine devices, intrauterine hormone-releasing systems, bilateral tubal occlusion, or vasectomized partners) during the study and for at least 18 months following the last dose of study drug. Women of non-childbearing potential are those who are postmenopausal greater than 2 year or who have had a bilateral tubal ligation or hysterectomy. Men who have partners of childbearing potential must agree to use an effective contraceptive method (condom or vasectomy) during the study and for at least 18 months following the last dose of study drug. Egg and sperm donation should be refrained for at least 18 months from the last dose of study drug, respectively. Exclusion Criteria: 1\. Major surgery within 4 weeks prior to the first dose of study drugs 2. Uncontrolled active systemic infection 3. Known positive serology for human immunodeficiency virus (HIV) 4. Patients with serologic evidence of prior vaccination (HBsAg negative, anti-HBs antibody positive, anti-HBc antibody negative) are eligible. Patients who are HBsAg negative/HBsAb positive but HBcAb positive are eligible, provided HBV DNA is negative and they are willing to take appropriate antiviral prophylaxis 5. Patients can be enrolled later if cleared by cardiology and repeat QTcF less than 480 msec. QTcF is calculated using Fridericia's Formula (QTcF): QTcF = QT/(RR0.33) 1. Correction of suspected drug-induced QTcF prolongation can be attempted at the investigator's discretion and only if clinically safe to do so with either discontinuation of the offending drug or switch to another drug not known to be associated with QTcF prolongation. 2.
