About this study
In plain language
This study tests ways to help rural primary care clinics refer more patients to Diabetes Self-Management Education and Support (DSMES) and help more patients use it. DSMES helps people learn to manage diabetes. The study is for clinic team members who help put these changes in place.
A multicomponent implementation strategy bundle designed to improve Diabetes Self-Management Education and Support (DSMES) referral and utilization in rural primary care clinics. The initial bundle includes: (1) establishing electronic health record (EHR) referral systems, (2) creating patient face sheets for providers, (3) discussing DSMES referrals during clinician and staff implementation team meetings, and (4) developing DSMES-specific marketing materials and promotional activities.
Compensation is read from the study's own description and may be missing — always confirm with the study team.
