What to expect

No one can promise a trial will help you personally — that is what the research is trying to find out.

What participation involves

Not stated in the study description — ask the study team about visits and procedures.

Compensation
Not stated
  • Study-related exams and procedures are typically provided at no cost. This is how many trials work in general — confirm the details for this study with the study team.
  • Participants are monitored closely by the research team throughout the study.
  • Some participants may receive access to an investigational treatment, which is still being studied.
  • Your participation helps researchers learn whether this approach could help future patients.

About this study

In plain language

This study looks at tailoring a high blood pressure program called TM-HTN for Black patients facing social disadvantages. It focuses on patients whose blood pressure stays high while taking stable blood pressure medicines.

Current clinic-based hypertension (HTN) management models have several limitations, resulting in episodic care that does not adequately support patients' self-care skills, and fails to achieve blood pressure (BP) control.

Compensation is read from the study's own description and may be missing — always confirm with the study team.

Eligibility

Who can join

  • Must be African American or Black.
  • Must face social disadvantages.
  • The top blood pressure number must have been 140 or higher at the last two clinic visits. At the study's start, the average of two readings taken by trained staff must be above 130.
  • Must have been on stable blood pressure medicines for the past 6 weeks.

Who can't join

  • Cannot read or speak English.
  • Have limited ability to measure blood pressure at home.
  • Have stage 4 or higher chronic kidney disease, meaning severe long-term kidney damage.
  • Have ongoing atrial fibrillation, a type of irregular heartbeat.
  • Have very high blood pressure, above 180/110.
  • Have had sudden health changes in the past 3 months that make blood pressure more likely to be unstable.
  • Have a terminal illness, meaning an illness expected to cause death.

Simplified for readability. The official criteria below are what the study team uses.

Official eligibility criteria

Inclusion Criteria: * Subjects must be African American or Black * experiencing socially disadvantage * Patients with systolic Blood Pressure ≥140 mmHg on their last two clinic visits and baseline systolic BP \>130 mmHg using the mean of two research BP values measured by trained staff * Subjects must be on stable Blood Pressure medications for the preceding 6 weeks Exclusion Criteria: * Unable to read or speak English * diminished ability to measure home Blood Pressure * chronic kidney disease ≥stage 4 * persistent/chronic atrial fibrillation * severe hypertension \>180/110 mmHg * acute health changes in past 3 months increasing chance of Blood Pressure instability * terminal illness
