What to expect
No one can promise a trial will help you personally — that is what the research is trying to find out.
What participation involves
- Visits mentioned: 8 visits
- Time periods mentioned: 8 weeks
- Procedures mentioned: Blood draws, Pills
Only what the study's summary and detailed description on ClinicalTrials.gov say. Time periods may describe the whole study or a part of it — ask the study team what applies to you.
Show the study's own words
- "The population to be included in this study are patients with depression and symptoms of anhedonia who have high inflammation as determined by a blood test."
- "Patients enrolled in the study will be randomly treated with either baricitinib or a placebo (matching sugar pill) for 8 weeks and assessed for markers of inflammation in the blood and symptoms of depression using self-reported and clinician-guided assessments."
- "Participation in the study will include at least 8 visits over 2-3 months, including screening."
- Compensation
- Not stated
- Study-related exams and procedures are typically provided at no cost. This is how many trials work in general — confirm the details for this study with the study team.
- Participants are monitored closely by the research team throughout the study.
- Some participants may receive access to an investigational treatment, which is still being studied.
- Your participation helps researchers learn whether this approach could help future patients.
About this study
In plain language
This study tests whether baricitinib, a drug that blocks JAK cell signals, can improve brain connections, motivation, and movement. It is for adults ages 25–55 with depression, high inflammation, and trouble feeling interest or pleasure.
This study will test the hypothesis that Janus kinase (JAK) signaling is involved in major depression (MD) with high inflammation by determining whether its inhibition with baricitinib can improve functional connectivity in reward and motor circuits in association with improved motivation and motor function in MD patients enriched for high C-reactive protein (CRP) and anhedonia.
Compensation is read from the study's own description and may be missing — always confirm with the study team.
Eligibility
Who can join
- Can and will give written consent to join.
- Are 25–55 years old.
- Have current major depression or the depressed type of bipolar disorder, confirmed by a study interview.
- Score above 14 on the PHQ-9 and at least 18 on the HAM-D, two depression rating tools.
- Have been off antidepressants and other mental health medicines for at least 4 weeks, or 8 weeks for fluoxetine.
- Have a CRP blood test result of at least 3 mg/L, a sign of inflammation.
- Score at least 2 on the PHQ-9 question about loss of interest or pleasure.
Who can't join
- Have an immune system disease that attacks the body, hepatitis B or C, HIV, or a history of cancer needing more than minor surgery.
- Have unstable major medical illness, certain low blood cell counts, liver scarring, certain abnormal liver tests, or kidney function below the study limit.
- Have a history of heart or blood vessel disease, stroke, a deep vein blood clot, or PML, a serious brain infection. Controlled high blood pressure is allowed.
- Have a current or recent infection, shingles symptoms within 12 weeks of study assignment, or past shingles that spread or caused complications. Major surgery within 8 weeks before screening or planned during the study also rules you out.
- Have certain disorders or symptoms involving loss of touch with reality, substance abuse or dependence within 6 months, an active suicide plan, or an active eating disorder. Some people with mood-linked binge eating may join.
- Have a history of a thinking or memory disorder, or a head injury that caused loss of consciousness. Are pregnant, breastfeeding, or using gender-affirming hormones.
- Use certain medicines that reduce inflammation or weaken the immune system, including regular NSAID pain relievers, steroids, or minocycline, within the study's time limits. Anti-inflammatory supplements are also restricted. Aspirin at 81 mg is allowed.
- Cannot safely have an MRI scan, cannot fit comfortably in the scanner, or have a BMI above 45, a body-size measure. More than 3 unsuccessful antidepressant trials during this episode or 5 over a lifetime also rules you out.
Simplified for readability. The official criteria below are what the study team uses.
