What to expect

No one can promise a trial will help you personally — that is what the research is trying to find out.

What participation involves

Not stated in the study description — ask the study team about visits and procedures.

Compensation
Not stated
  • Study-related exams and procedures are typically provided at no cost. This is how many trials work in general — confirm the details for this study with the study team.
  • Participants are monitored closely by the research team throughout the study.
  • Some participants may receive access to an investigational treatment, which is still being studied.
  • Your participation helps researchers learn whether this approach could help future patients.

About this study

In plain language

This study tests low-dose cyclophosphamide with tacrolimus and ruxolitinib after a donor stem cell transplant. It aims to prevent graft-versus-host disease (GVHD), when donor cells attack the body. It is for adults with certain blood cancers who will get strong treatment before the transplant.

This is an open label, non-randomized, multicenter, pilot, dose expansion study of low dose post-transplant cyclophosphamide (25 mg/kg on Days +3 and +4)/tacrolimus/ruxolitinib in the setting of myeloablative conditioning (MAC) allogeneic peripheral blood stem cell transplantation (PBSCT).

Compensation is read from the study's own description and may be missing — always confirm with the study team.

Eligibility

Who can join

  • Be age 18 or older and freely sign a consent form before study procedures.
  • Have acute leukemia with no blasts (young cancer cells) in the blood and less than 5% in the bone marrow; or have myelodysplasia (MDS) or chronic myelomonocytic leukemia (CMML), with no blood blasts and less than 10% in the marrow.
  • Plan to get strong treatment to prepare for a transplant using stem cells from a donor’s blood. The donor must be a sibling with a 6/6 tissue match or an unrelated person with an 8/8 match, and meet donation rules.
  • Meet heart, kidney, and lung test limits: heart pumping strength at least 45%, kidney clearance above 60 mL/min, and both required lung tests at least 60%.
  • Have liver enzymes below 3 times the upper normal limit and bilirubin below 2 mg/dL, with exceptions for Gilbert’s syndrome or red blood cell breakdown. Have an activity score of at least 70%, meaning you can care for yourself.
  • Women who can become pregnant must use two effective birth control methods or avoid sex with men from consent until 12 months after transplant.
  • Men with partners who can become pregnant must use effective barrier birth control or avoid sex with women during that same period, even after surgery to prevent pregnancy.
  • Tell the study team about planned medicines to keep cancer under control after transplant. Some targeted medicines are allowed; experimental medicines and medicines called hypomethylating agents are not.

Who can't join

  • A past donor stem cell transplant; cancer in the brain or spinal cord; or acute myeloid leukemia that arose from certain marrow diseases, including CMML or MDS/MPN. Leukemia arising from MDS alone is allowed.
  • An infection that is getting worse or not improving with treatment; active or poorly treated inactive TB; detectable HIV; or uncontrolled hepatitis B or C. Controlled HIV or hepatitis may be allowed under study rules.
  • A blood clot, stroke, or heart attack within 6 months, except a clot linked to a catheter. Severe heart failure, uncontrolled chest pain, severe uncontrolled heart rhythms, signs of poor heart blood flow, or inherited long QT syndrome also exclude you.
  • Being pregnant or breastfeeding, or having a serious physical or mental illness that could prevent taking part.
  • A past cancer, unless it meets an exception. Removed non-melanoma skin cancer, treated early cervical cancer, and cancer treated to cure it at least 5 years ago are allowed. More recent cancers need study approval and must meet specific rules.
  • Plans to use ATG or alemtuzumab before transplant, or to receive donor white blood cells to prevent problems after transplant.
  • Any past use of ruxolitinib.
  • Use of immune checkpoint inhibitors, medicines that help the immune system attack cancer, within 6 months before transplant preparation.

Simplified for readability. The official criteria below are what the study team uses.

Official eligibility criteria

Inclusion Criteria: * Age 18.0 years or older at the time of enrollment * Patients undergoing allogeneic hematopoietic cell transplantation for one of the following indications: * Acute leukemia with no circulating blasts and with less than 5% blasts in the bone marrow * Myelodysplasia/chronic myelomonocytic leukemia with no circulating blasts and with less than 10% blasts in the bone marrow (higher blast percentage allowed in MDS due to lack of differences in outcomes with \<5% versus 5-10% blasts in this disease). * Planned myeloablative (MAC) conditioning regimen (see eligible regimens in Section 9.2) * Patients must have a related or unrelated peripheral blood stem cell donor as follows: * Sibling donor must be a 6/6 match for HLA-A and -B at intermediate (or higher) resolution, and -DRB1 at high resolution using DNA-based typing and must be willing to donate peripheral blood stem cells and meet institutional criteria for donation. * Unrelated donor must be an 8/8 match at HLA-A, -B, -C and -DRB1 at high resolution using DNA-based typing. Unrelated donor must be willing to donate peripheral blood stem cells and meet National Marrow Donor Program (NMDP) criteria for donation. \* Donor selection must comply with 21 CFR 1271 * Cardiac function: Left ventricular ejection fraction at least 45% * Estimated creatinine clearance greater than 60 ml/min (C-G formula) * Pulmonary function: DLCO (diffusing capacity of lung for carbon monoxide) corrected for hemoglobin at least 60% and FEV1 (forced expiratory volume at one second) predicted at least 60% * Liver function: AST(Aspartate aminotransferase)/ALT(Alanine aminotransferase) \<3x Upper Limit of Normal (ULN); Total bilirubin \<2 mg/dL excluding Gilbert's syndrome or hemolysis * Karnofsky Performance Score at least 70%. * Female patients (unless postmenopausal for at least 1 year before the screening visit, or surgically sterilized), agree to practice two (2) effective methods of contraception at the same time, or agree to completely abstain from heterosexual intercourse, from the time of signing the informed consent through 12 months post-transplant. * Male patients (even if surgically sterilized), of partners of women of childbearing potential must agree to one of the following: practice effective barrier contraception or abstain from heterosexual intercourse from the time of signing the informed consent through 12 months post-transplant. * Plans for the use of targeted small molecule inhibitor post-transplant maintenance therapy must be disclosed upon enrollment. Planned use of investigational maintenance agents is not permitted. Planned hypomethylating agents as maintenance therapy is not permitted. Allowed maintenance includes: * FLT3 inhibitors: gilteritinib, sorafenib, midostaurin * IDH inhibitors: enasidenib, ivosidenib * BCR/ABL inhibitors: imatinib, ponatinib, dasatinib, nilotinib * Other targeted therapies may be discussed with protocol chairsVoluntary written consent obtained prior to the performance of any study-related procedure that is not a part of standard medical care, with the understanding that consent may be withdrawn by the patient at any time without prejudice to future medical care. * There are no restrictions based on blood counts as this intervention is being used in combination with intensive chemotherapy with intent to myeloablate. Exclusion Criteria: * Prior allogeneic transplant * Active CNS (central nervous system) involvement by malignant cells * Patients with secondary acute myeloid leukemia arising from myeloproliferative neoplasms or overlap syndromes, including CMML(chronic myelomonocytic leukemia) and MDS/MPN (myelodysplastic syndromes/myeloproliferative neoplasms) syndromes; patients with secondary acute myeloid leukemia arising from myelodysplastic neoplasm are eligible. * Patients with uncontrolled bacterial, viral, or fungal infections (currently taking medication and with progression or no clinical improvement) at time of enrollment. * Active or inadequately treated latent infection with Mycobacterium tuberculosis (i.e., TB). * Patients seropositive for human immunodeficiency virus (HIV) with detectable viral load.
