What to expect

No one can promise a trial will help you personally — that is what the research is trying to find out.

What participation involves

Not stated in the study description — ask the study team about visits and procedures.

Compensation
Not stated
  • Study-related exams and procedures are typically provided at no cost. This is how many trials work in general — confirm the details for this study with the study team.
  • Participants are monitored closely by the research team throughout the study.
  • Some participants may receive access to an investigational treatment, which is still being studied.
  • Your participation helps researchers learn whether this approach could help future patients.

About this study

In plain language

This study tests how well LAM-001 works and how safe it is compared with a placebo, a treatment with no active drug. LAM-001 is sirolimus breathed into the lungs. The study is for adults with sarcoidosis, a disease that causes swelling in organs, and related high blood pressure in the lungs.

A Phase 2, Randomized, Double-Blind, Multi-Dose, Placebo-Controlled Study to Evaluate the Efficacy and Safety of LAM-001 (Inhaled Sirolimus) in Adults with Sarcoidosis Associated Pulmonary Hypertension (SAPH)

Compensation is read from the study's own description and may be missing — always confirm with the study team.

Eligibility

Who can join

  • Be 18–80 years old. People over 70 need approval from the study’s medical monitor.
  • Have sarcoidosis confirmed by a tissue sample and lung blood pressure tests that meet the study’s limits.
  • Have symptoms that cause mild or marked limits on physical activity, called WHO class II or III.
  • Have a breathing test result called FVC of at least 50% of the expected value.
  • If taking medicines for lung blood pressure or sarcoidosis swelling, doses must be stable for at least 3 months, with no new medicines started within 6 months. Symptoms must remain despite treatment. These medicines are not required.
  • Walk 100–450 meters in 6 minutes on two screening tests. The results must be within 15% of each other.
  • Understand and sign the consent form, follow study rules, and attend study visits.
  • Follow pregnancy testing and birth control rules. Women who can become pregnant need two negative tests before treatment and monthly tests. Required birth control and limits on breastfeeding and blood, egg, or sperm donation continue through 90 days after treatment.

Who can't join

  • Certain active sarcoidosis problems affecting the brain, nerves, or heart; other causes of high lung blood pressure; certain heart test results; or three or more listed heart risk factors.
  • Severely reduced breathing or oxygen transfer on lung tests, oxygen use of 6 or more liters per minute while resting, inability to do the walk test, or untreated severe sleep apnea, which causes pauses in breathing.
  • A serious illness that could prevent participation, expected survival under 2 years, or certain high liver test results or severe kidney problems.
  • Pregnancy, breastfeeding, or not following the required birth control rules.
  • Recent use of an oral mTOR blocker, a type of medicine that includes sirolimus; another study drug or device within 4 weeks; or a severe allergy to sirolimus or lactose ingredients.
  • Major surgery within 60 days before treatment or not recovered from surgery. Cancer within the past 5 years also excludes people, except certain fully treated skin or early cervical cancers.
  • Smoking or vaping, current alcohol use disorder, or a positive test for amphetamines, methamphetamines, or cocaine in the past 6 months.
  • Active HIV or hepatitis C, active or untreated inactive tuberculosis, or a lung infection needing treatment.

Simplified for readability. The official criteria below are what the study team uses.

Official eligibility criteria

Inclusion Criteria: Participants must satisfy all the following criteria to be enrolled in the study: 1. Patients 18-80 years (\>70 requires medical monitor approval) with diagnosis of sarcoidosis based on American Thoracic Society (ATS)/World Association of Sarcoidosis and Other Granulomatous Disorders (WASOG) criteria 2. SAPH with precapillary phenotype diagnosed by mean pulmonary arterial pressure (mPAP) \> 20 mmHg, pulmonary capillary wedge pressure (PCWP) ≤ 15 mmHg, PVR ≥ 4.0 Wood units (WU) as determined by the RHC core lab 3. WHO functional class (FC) II or III 4. Confirmed positive sarcoidosis biopsy from any affected organ 5. FVC ≥ 50% predicted 6. If on background PH and anti-inflammatory sarcoidosis therapies, must be stable for at least 3 months and patient should be symptomatic despite background therapy. No new PH or anti-inflammatory sarcoidosis drugs started within 6 months. PH or sarcoidosis therapies are not required for inclusion. 7. 6MWD ≥ 100 and ≤ 450 meters repeated twice during Screening Period and both values within 15% of each other, calculated from the highest value 8. Subjects must be able to understand and be willing to sign the written informed consent form. A signed informed consent form must be appropriately obtained prior to the conduct of any trial-specific procedure. 9. Women of childbearing potential must satisfy the following: 1. Have 2 negative pregnancy tests as verified by the investigator prior to starting study and must agree to ongoing monthly pregnancy testing during the study and at end of study treatment. 2. If sexually active, must have used, and agree to continue to use, highly effective contraception without interruption, for at least 30 days prior to starting investigational product (IP), during the study (including dose interruptions), and for 90 days after discontinuation of study treatment. 3. Refrain from breastfeeding a child or donating blood, eggs, or ovum for the duration of the study and for at least 90 days after the last dose of study treatment 10. Male participants must use highly effective forms of contraception: 1. Agree to use a condom, defined as a male latex condom or nonlatex condom NOT made from natural (animal) membrane (for example, polyurethane), during sexual contact with a pregnant female or a female of childbearing potential while participating in the study, during dose interruptions, and for at least 90 days following IP discontinuation, even if he has undergone a successful vasectomy. 2. Refrain from donating sperm for the duration of the study and for 90 days after the last dose of study treatment. 11. Ability to adhere to the study visit schedule and understand and comply with all protocol requirements. 12. Ability to understand and provide written informed consent Exclusion Criteria: Participants will be excluded from the study if they meet any of the following criteria: 1. Patients receiving systemic immunosuppressive therapy to treat neurosarcoidosis or therapy for active neurological disease or other forms of sarcoidosis likely to impact life expectancy over the 12 months of the study. Participants with a history of neurosarcoidosis who have not required new or increased immunosuppressants in the past 6 months are not excluded. 2. Patients with cardiac sarcoidosis receiving treatment for active cardiac disease. Patients not on treatment are not excluded. 3. Left ventricular ejection fraction (LVEF) \<50%, within the previous 6 months with Left atrial volume index greater than 45 4. Patients with a PCWP \> 15 mm Hg determined at end exhalation by the RHC Core Lab 5. Patients with other causes of Pulmonary Hypertension including CTEPH and Group 2 PH. A CT scan with contrast within the last 3 months and/or a V/Q scan if clinically indicated are sufficient to rule out other causes of PH. 6. Patients with an FVC \<50% of predicted during the screening visit 7. Patients with severe airway obstruction as defined by 1-second forced expiratory volume (FEV1)/FVC \<50% absolute during screening visit 8. DLCO \<25% predicted 9.
