Neladalkib (NVL-655) for TKI-naive Patients With Advanced ALK-Positive NSCLC

A Phase 3 Study of the Selective Anaplastic Lymphoma Kinase (ALK) Inhibitor NVL-655 Compared to Alectinib in First-Line Treatment of Patients With ALK-Positive Advanced Non-Small Cell Lung Cancer (ALKAZAR)

What to expect

No one can promise a trial will help you personally — that is what the research is trying to find out.

What participation involves

Not stated in the study description — ask the study team about visits and procedures.

Compensation
Not stated
  • Study-related exams and procedures are typically provided at no cost. This is how many trials work in general — confirm the details for this study with the study team.
  • Participants are monitored closely by the research team throughout the study.
  • Some participants may receive access to an investigational treatment, which is still being studied.
  • Your participation helps researchers learn whether this approach could help future patients.

About this study

In plain language

This study compares neladalkib (NVL-655) with alectinib to see which keeps lung cancer from growing or spreading longer. It is for people with advanced non-small cell lung cancer (NSCLC) that has a change in the ALK gene and who have not had ALK-targeted drugs or most other drug treatments for NSCLC.

Multicenter, randomized, controlled, open-label, Phase 3 study designed to demonstrate that neladalkib (NVL-655) is superior to alectinib in prolonging progression-free survival (PFS) in patients with treatment-naïve, Anaplastic Lymphoma Kinase (ALK) positive, advanced Non-Small Cell Lung Cancer (NSCLC).

Compensation is read from the study's own description and may be missing — always confirm with the study team.

Eligibility

Who can join

  • Have NSCLC confirmed by tissue or cell testing. The cancer has spread to distant parts of the body, or is locally advanced and cannot be treated with a mix of treatment types.
  • Have an ALK gene change confirmed by a tissue or blood test.
  • Have not had cancer drugs that treat NSCLC throughout the body. Chemotherapy before or after surgery is allowed if given at least 12 months before study group assignment.
  • Have never taken an ALK-blocking drug, such as alectinib.
  • Have at least one tumor that can be measured using the study’s rules.
  • Have a tumor tissue sample from before treatment.

Who can't join

  • The cancer has another known gene change that drives its growth besides ALK.
  • Have a known allergy to any inactive ingredient in neladalkib or alectinib.
  • Are getting radiation treatment or had it too recently under study rules, or had major surgery within 4 weeks before study group assignment.
  • Have an uncontrolled infection needing medicine that works throughout the body, active tuberculosis, or active hepatitis B or C.
  • Repeated heart tests show a QTcF above 470 milliseconds, meaning the heart takes too long to reset between beats.
  • Have significant heart or blood vessel disease.
  • Cancer in the brain is causing worsening nerve or brain symptoms, or needs increasing steroid doses to control it.
  • Have an active cancer that needed treatment within 2 years before study group assignment.

Simplified for readability. The official criteria below are what the study team uses.

Official eligibility criteria

Inclusion Criteria: 1. Histologically or cytologically confirmed locally advanced (not amenable for multimodality treatment) or metastatic Non-small Cell Lung Cancer (NSCLC) 2. Documented Anaplastic Lymphoma Kinase (ALK) rearrangement via testing of tissue or blood 3. No prior systemic anticancer treatment for NSCLC (adjuvant/neoadjuvant chemotherapy allowed if 12 months prior to randomization; prior ALK tyrosine kinase inhibitor \[TKI\] such as alectinib is not allowed in any setting) 4. Measurable disease (1 or more target lesions per Response Evaluation Criteria in Solid Tumors \[RECIST\] 1.1) 5. Pretreatment tumor tissue Exclusion Criteria: 1. Patient's cancer has a known oncogenic driver alteration other than ALK. 2. Known allergy/hypersensitivity to excipients of neladalkib or alectinib. 3. Ongoing or recent radiotherapy as per protocol-specified timeframes prior to randomization 4. Major surgery within 4 weeks prior to randomization 5. Uncontrolled clinically relevant infection requiring systemic therapy 6. Known active tuberculosis, or active Hepatitis B or C 7. QT corrected for heart rate by Fridericia's formula (QTcF) \> 470 msec on repeated assessments 8. Clinically significant cardiovascular disease 9. Brain metastases associated with progressive neurological symptoms or requiring increasing doses of corticosteroids to control CNS disease 10. Active malignancy requiring therapy within 2 years prior to randomization
