About this study
In plain language
This study tests how well teplizumab works and how safe it is compared with a placebo, a treatment with no active drug. It is for people ages 1 to 25 with recently diagnosed stage 3 type 1 diabetes who use standard insulin treatment. Both treatments are given through a vein, and researchers will track blood sugar control and whether people can stop mealtime insulin over 52 weeks.
This is a multicenter, randomized, double-blind, parallel, placebo-controlled Phase 3, 2-arm study for treatment. The purpose of this study is to measure change in glycemic control and prandial insulin independency over 52 weeks with teplizumab compared with placebo, both administered by intravenous (IV) infusion, in participants with recently diagnosed Stage 3 type 1 diabetes (T1D) aged 1 to 25 years, on standard insulin therapy.
Compensation is read from the study's own description and may be missing — always confirm with the study team.
Eligibility
Who can join
- Be 1 to 25 years old when the consent form is signed. Males and females may join.
- Have stage 3 type 1 diabetes based on the required medical guidelines, and start study treatment within 8 weeks of diagnosis.
- Have at least one required type 1 diabetes autoantibody, a sign that the body's defenses attack its own cells.
- Have a C-peptide blood level of at least 0.2 nmol/L. This test shows how much insulin your body makes.
- Not be pregnant. If able to become pregnant, have negative pregnancy tests at screening and within 24 hours before the first dose.
- If able to become pregnant, use highly effective birth control before treatment starts, during treatment, and for at least 30 days after the last dose.
- If breastfeeding, stop and pump and throw away breast milk during treatment and for 20 days after the last dose.
- Give signed consent and be able to follow study rules. For children, a legally authorized representative must also sign.
Who can't join
- Have another type of diabetes instead of type 1 diabetes caused by the body's defenses attacking insulin-making cells.
- Have a serious active infection or fever of at least 101.3°F within 48 hours before the first dose, except a local skin infection; or have a long-lasting, repeated, or unusual infection.
- Have active Epstein-Barr virus or cytomegalovirus infection, positive HIV or hepatitis B or C tests, or tuberculosis, including tuberculosis that is not causing symptoms.
- Have another disease in which the body's defenses attack healthy tissue, except stable autoimmune thyroid disease or controlled celiac disease if the study doctor allows it. Other health problems that make the study unsafe or affect results may also exclude you.
- Have live vaccines within 8 weeks before treatment, during treatment, or within 26 weeks after a treatment course. Have inactivated or mRNA vaccines within 2 weeks before treatment, during treatment, or within 6 weeks after a treatment course.
- Use certain medicines now or too recently, including diabetes drugs other than insulin, drugs that affect blood sugar or the immune system, or drugs that interfere with study treatment. Any past teplizumab or other anti-CD3 treatment also excludes you.
- Be in another study now, or have received an experimental treatment too recently: within 8 weeks before screening or longer if the treatment stays in the body longer.
- Have blood counts below the study's limits or liver blood tests above its limits. Some exceptions apply for a genetic trait called Duffy-null or a condition called Gilbert's syndrome.
Simplified for readability. The official criteria below are what the study team uses.
Official eligibility criteria
Inclusion Criteria: * Participants are eligible to be included in the study only if all of the following criteria apply: * Participant must be 1 to 25 years of age inclusive, at the time of signing the informed consent. * Participants diagnosed with T1D Stage 3 according to American Diabetes Association 2025 criteria * Participants able to be randomized and initiate study drug within 8 weeks (56 days) of the Stage 3 T1D diagnosis * Participants must be positive for at least one T1D autoantibody at screening: * Glutamic acid decarboxylase (GAD-65), * Insulinoma Antigen-2 (IA-2), * Zinc-transporter 8 (ZnT8), or * Insulin (if obtained not later than 14 days after exogenous insulin therapy initiation). * Islet cell cytoplasmic autoantibodies (ICAs) * Have random C-peptide level ≥0.2 nmol/L obtained at screening * Enter Inclusion Criteria Sex * Both male and female participants are eligible. * Contraceptive use by women should be consistent with local regulations regarding the methods of contraception for those participating in clinical studies. * A female participant is eligible to participate if she is not pregnant, and one of the following conditions applies: * Is a woman of nonchildbearing potential (WONCBP) OR * Is a woman of childbearing potential (WOCBP) and agrees to use a contraceptive method that is highly effective, with a failure rate of \<1% during the study intervention period (to be effective before starting the intervention) and for at least 30 days after the last administration of study intervention. * A WOCBP must have a negative highly sensitive pregnancy test at screening (serum) and within 24 hours (urine or serum as required by local regulations) before the first administration of study intervention. * Lactating woman must interrupt breastfeeding and pump and discard breast milk during and for 20 days after last administration of study intervention. * Capable of giving signed informed consent as described in Appendix 1 of the protocol which includes compliance with the requirements and restrictions listed in the informed consent form (ICF) and in the protocol.
